We don't prefer to give away the affiliate page code but I
can guide you to make something similar to it.

Navigate to your
Admin CP > Modules > HTML Page Management > Create new page.
For "Do you wish to use your forum header and footer ?", select "Yes".

Now,
in the HTML source area put the codes that Kratos gave you above and
save the page. Now, you can copy the URL of that page and out a custom
link in your nav bar by going to Admin CP > Display > Headers
& Navigation.
